Students participating in interscholastic athletics, cheerleading and dance; and their parents/guardians; must sign the acknowledgement below and return it to their school. Students cannot practice or compete in those activities until this form is signed and returned.
The role of the CCA Booster Club is to help in providing equipment designed to boost our athletic programs and the performance of CCA athletes. We work in partnership with all of the CCA Athletic Programs trying to meet their many needs. The CCA Booster Club may not be able to fully fund all of your allocation request(s), therefore, fundraising or club funds will need to be used for the purchase. The CCA Booster Club Allocation of Funds meeting is held every June. We encourage all coaches to attend. For consideration of funds, a completed Allocation Request form along with supporting documentation must be submitted prior to the June 9th meeting. All Allocation Requests will be reviewed. This form is to be completed by coaches. New requests will be considered throughout the school year as needs arise.

Senior citizens: Free, must be resident of CCA district
